This form is used by league officials to record and track fines
Game change caused by State Cup or ODP
Use this form to REQUEST a game change caused by State Cup or ODP. All REQUESTS will be reviewd by the league scheduler
Game Change Request - During Preliminary Period
Use this form to REQUEST a game change during the tentative schedule period. All REQUESTS will be reviewed by league scheduler
Game Change Form Caused By Weather
Use this form to REQUEST a rescheduled game due to weather. All REQUESTS will be reviewed by the league scheduler
WNYR Syracuse District Referee Report of CNYJSA League Games
This form is only used by Referee's after each game.
CNYJSA Coach's Comment Form
Use this form to notify the league of any coach, player or spectator incident that needs to be reviewed
